ADMISSION CRITERIA

Persons who live in Helderberg basin. Persons over 12 years of age

Persons diagnosed with a life threatening illness such as: Cancer, Motor Neuron Disease, Multiple Sclerosis, End stage Chronic Obstructive Pulmonary Disease, End stage organ failure, HIV/TB, End stage Parkinsons

ADMISSION PROCESS

Patient’s requiring specialist palliative care from Helderberg Hospice are generally referred to our service by their treating medical practitioner. All referrals for service are assessed by our Inter-Disciplinary Team and individually assessed for suitability.

Medical Professionals, patients and family members may also request referral forms by e-mailing us on nursing@helderberghospice.org.za. Once we receive your completed referral forms, we promise to get back to you within 48hrs to advise you of the status of your referral.

OTHER WAYS WE CAN HELP YOU

Home Care is our core service and all patients accepted into our home care programme must be formally referred by their doctor, hospital or clinic. Referral forms are available from the referring doctor. LEARN MORE

The 24-hour Palliative Care Support Centre at Helderberg House is a 10-bed facility offering 24 hours nursing and is used primarily as back-up for the home care service. It is licensed by the Department of Health and accredited with the Council of Health Service Accreditation of Southern Africa (COHSASA). Care is provided by a professional palliative nurse sister and two trained auxiliary nurses on each shift, day and night. Patients are admitted from home for pain and symptom management, family respite and terminal care. LEARN MORE

Patients and their families who are affected by terminal illness are confronted by a number of difficulties which may include family conflict, unresolved relationships, depression, anger, and anxiety. Added to this are worries about practical issues such as financial strain, employment, future care, and care of minor children in the family.  It is important to note that our social workers do not remove children, but that they work together with families to ensure timeous planning for the care of potential orphans and vulnerable children.  Should children be in need of alternative care, our social workers refer to child and family welfare organisations in the area for further follow-up and intervention. Anyone who has lost a loved one knows that the intense emotions associated with grief can be overwhelming and traumatic. Our social workers together with our trained volunteer bereavement supporters, provide bereavement services and support to family members, to assist them in coping with the death and in making the necessary adjustments required in order to continue living without their loved one.
